ADMIN PER SE SUBSTANCE ABUSE SCREENINGS:
This screening is required by MVD for individuals who have been pulled over for any alcohol or drug related driving tickets. The appointment will take 45 minutes on average to complete. You will need to bring a copy of your ticket or the admin per se from the MVD. A licensed counselor will do the screening and you will receive a print out from MVD confirming the receipt of the evaluation before you leave our office.

MVD REVOCATION SUBSTANCE ABUSE SCREENING:

This screening is required for individuals with a revoked license. It is to be completed upon eligibility to drive. This appointment will take around one hour to complete. You will need to bring:

• Your revocation packet with page "A" completed and notarized WITHIN THE PAST 30 DAYS, and "B" completed by the court where the last alcohol or drug related ticket was issued. If your last offense was a felony, have your probation officer fill it out or take it to the probation department to fill out. SUPERIOR COURT WILL NOT FILL YOUR PART "B". Bring part "C" blank. OUR OFFICE WILL MAIL OUT THE PACKET IN ITS ENTIRETY.

Your screening will be mailed to the MVD from our office after your appointment and the MVD will mail you a copy of their decision 3-4 weeks after your appointment. MVD DETERMINES WHETHER YOU HAVE YOUR LICENSE REINSTATED!
